Student Pre-participation Physicals Update
Houston Methodist West Orthopedics and Sports Medicine will be hosting an additional physical date on May 22nd from 8 a.m. – Noon by appointment only.
Please contact Brittany Loyd at bgloyd@houstonmethodist.org or 832-840-4638 to schedule your appointment. You will need a copy of the schools physical form, a filled out medical history, and a completed Houston Methodist Consent form which can be found on the athletics page of the school website.
